I understand that these photos and/or videos volition be the belongings of the attending doc, and may be used for, amidst other purposes, medical pedagogy. The physician has answered all of my questions regarding this procedure. I certify that I have read and empathize this treatment agreement and that all blanks were filled in prior to my signature.
